Richards PTO Reimbursement Request Form 2019-2020

IMPORTANT: After submitting this form please do one of the following:

  1. Email a photo of your receipt to richards.treasurer@gmail.com OR

  2. Place your original receipt with a note indicating the name of the committee/event the receipt is for in the treasurer’s mail box in the copy room of the Richards Main Office.

No reimbursements will be issued without receipts

The PTO is a tax-exempt organization – no sales tax should be paid.  Use Richards’ tax-exempt certificate when purchasing items you will submit receipts for.
